Long Term Conditions
If you have a long term condition, like diabetes or asthma, our team will help you manage your condition. Everyone with a long term condition can expect to be invited to a yearly review to monitor their condition and help them manage it.

Screening Test
Screening is a way of finding out if people are at higher risk of a health problem, so that early treatment can be offered or information given to help them make informed decisions.
Pregnancy and Children
If you think you are pregnant please make an appointment as soon as possible with your GP, who will share your care with the local midwife team. After your child is born we will continue to monitor their development and immunise them as and when necessary.
Diabetic Clinic
Diabetes clinics give you the chance to meet medical experts who can check that you are as healthy as possible and give you highly specialised advice on how to look after your diabetes.
Oakwood surgery Dr Al Qazi is the expert GP in Diabetes. This is the doctor in charge of your care and treatment and if he then refer you to a diabetes clinic for specialist diabetes care. We provide once a year review to ask questions about your diabetes and get your vital diabetes checks.
Travel Vaccinations
Travel Vaccinations
If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible – at least 6 weeks before you travel – as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.

Health Checks
We can carry out NHS health checks to patients who are aged 40 to 74. We also invite selected patients for enhanced health checks.
